yaṃ yaṃ vāpi smaranbhāvaṃ tyajatyante kalevaram | taṃ tamevaiti kaunteya sadā tadbhāvabhāvitaḥ ||
Whatever state of being one remembers when at last leaving the body, to that very state one goes, son of Kunti, having always been absorbed in that state.
Never use this verse to tell a grieving person that their loved one's fate was decided by a frightened, confused or unconscious last moment; the verse itself points to a whole life ('always absorbed'), not one instant. Acknowledge the grief first. Not medical or end-of-life advice.
Meaning today and context
The key words are 'always absorbed': what fills the mind at the end is what we filled it with over a lifetime. So the verse is really about how we live each day, not about one final moment.
Krishna states the general principle behind 8.5: whatever state of being one remembers at the end, one goes to that, having been shaped by it throughout life.
